This study aimed to analyze the epidemiological profile of malaria in Amajarí between 2020 and 2025, integrating temporal, spatial, and clinical-parasitological dynamics with the socioenvironmental determinants of the region. The methodology consisted of a descriptive, observational, and retrospective epidemiological study with a quantitative approach, based on secondary data from the SIVEPMalaria and SINAN information systems. The results showed that, after a reduction in cases between 2020 and 2022 a period possibly affected by underreporting during the COVID-19 pandemic the municipality experienced a critical outbreak in 2023 and 2024. In 2024, the Annual Parasitic Incidence (API) reached a historic level of 608.31 cases per 1,000 inhabitants, consolidating the region as an area of extremely high risk. Transmission is predominantly autochthonous and concentrated in Indigenous Lands (Yanomami Indigenous Territory), with particular emphasis on the Auaris region, and Plasmodium vivax was identified as the most prevalent species. Illegal gold mining and deforestation were identified as the main drivers of the disease.
